By entering into the store and/or using the self-serve pet wash areas, you agree to waive all liability against Elite Pooch Boutique and its owners/agents for any injuries to you and your pet, damages, and lost items arising out of the use of the self-dog wash services.
We value you and your pet and want to ensure you that we will make every effort to make you and your pet’s visit as safe and enjoyable as possible. Everyone’s safety comes first at Elite Pooch Boutique, both the people and the dogs.
I hereby consent to using the self-dog wash facilities provided by Elite Pooch Boutique. I understand and agree to the following terms and conditions:
1. Assumption of Risk: I acknowledge that bathing a dog(s) involves inherent risks, including but not limited to slips, falls, and potential injuries to myself or my dog(s)
.2. Supervision: I agree to supervise my dog(s) at all times while using the self-dog wash facilities. I understand that Elite Pooch Boutique is not responsible for supervising my dog(s).
3. Health and Safety: I certify that my dog(s) is/are in good health and has no known medical conditions that would prevent them from being bathed. I agree to follow all safety guidelines provided by Elite Pooch Boutique
.4. Grooming (cutting & clipping): I understand that Elite Pooch Boutique is a dog wash facility only. I agree and understand that NO clipping, cutting or grooming; other than brushing my dog(s) is allowed and that trimming/cutting tools of any kind are strictly prohibited.
5. Flea and Tick: I certify that my dog(s) is free of fleas and ticks, and I agree to take appropriate measures to prevent the spread of fleas and ticks within Elite Pooch Boutique premises If parasites are found during bath we do have flea shampoo available for an additional cost
.6. Rabies and/or Titers: I certify that my dog(s) is current on their rabies vaccinations and/or has up-to-date titers testing. While we do not ask for verification we do ask all dogs be fully vaccinated for the safety of others
Unattended Dogs: I agree that at no time will I leave my dog(s) unattended while using the self-dog wash facilities, including in the tub. I understand the importance of constant supervision for the safety and well-being of my dog(s) and others.
Cameras: I hereby grant Elite Pooch Boutique, its owners, agents, employees, and affiliates consent to video you, your pet, or any person/s with you whether an adult or child. Our footage is backed up by a server and is held into a footage bank. Elite Pooch Boutique has cameras throughout our facility filming 24/7 to ensure not only our safety, but yours as well.
Photographs: I hereby release Elite Pooch Boutique, its owners, agents, employees, and affiliates from any liability to use photographs of my pet/s for use in customer files, as well as for the company’s website, advertising, and social media platforms.
Wet Floors: I am fully aware that the facility has self-dog wash stations and will more than likely have wet, damp, slick floors. I will proceed with caution while using the facility. I will not hold Elite Pooch Boutique. or its owners/agents liable for any accidental or non-accidental injuries. I will enter the facility with caution and at my own risk.
Liability Release: I hereby release Elite Pooch Boutique, its owners, agents, employees, and affiliates from any liability for any injuries or damages that may occur to myself, my dog(s), or any third parties while using the self-dog wash facilities.
